Great list! For de Nicolai, there's also 69 rue Poincare (Victor Hugo). From the store you can see the lab through a glass, and I once noticed the perfumer herself at a business meeting there. While not in the main shopping district, the posh residential area also has a Diptyque and a Malle (see below).

Other brands:
Diptyque (several, including 34, boulevard saint germain, or 49 Avenue Victor Hugo)
Montale/Tanelli 26 place Vendome (a store hated by many).
not french, but interesting, Arabian oud, 63 Champs Elysees (perhaps the only place in Europe outside of London where one can find good arabian oils).

Great list, and very handy to have all these must-sniff spots in one place.
I would add Marie-Antoinette, a tiny multi-brand outlet: http://marieantoinetteparis.fr/marieantoinetteparis/Accueil.html
and also JAR Parfums rue de Castiglione for the scents and the boudoir-setting, unlike any other.
My next trip won't come soon enough! ^_^

While not strictly an outlet for perfume Cire Trudon is a wonderful place for anyone interested in aroma. The oldest manufacturer of scented candles on earth.
78, rue de Seine - 75006 Paris

Again not a perfumery but for a most memorable olfactory experience Marriage Freres is hard to beat. Insinuating the scent of rare teas into everything from food to incense. Would that the air in the shop were for sale.
30 rue du Bourg-Tibourg, Paris 4e

l'artisan parfumeur twice at the marais (near sens unique):
32, rue du bourg tibourg and
34, rue francs
